The licensing inspector conducted an unannounced renewal inspection, at which play groups, outdoor time, story time, group time, personal hygiene, art time, lunch time, and rest time were observed. Children were observed; and staff and children files and other documentation were reviewed. One hundred fourteen (114) children, aged 4 months-5 years, were found to be in care with 18 staff. Previous violations were reviewed, and one (1) of the violations from the previous inspection had not been corrected. Please contact me if you need further assistance. Thank you for your cooperation. Standard #: 22VAC40-185-350-C Description: Based on interviews with staff and review of documentation the facility failed to maintain compliance with this standard as required relating to staff to child ratios. Evidence: On 2/22/19 there were a total of 23 children aged infant-3 years present with only 3 staff at 7:15am-7:30am. (5 infants, 4 toddlers, 7 two year olds, and 7 three year olds) Ratios required 1 additional staff to be working with the children. 2/22/19: 7:30am-7:45am there were 33 children aged infant-3 years with 5 staff. Ratios required 1 additional staff to be working with the children. 2/22/19: 9:15am-9:30am there were 123 children with 18 staff. The preschool classroom (4-5 year olds) had 22 children in care with 2 staff; the required ratio of 1 staff to 10 children was not being met. The toddler classroom 16-24 months had 11 children with 2 staff; the required ratio of 1 staff to 5 children was not being met. 2/22/19: 9:30am-2:45pm the 2 year old class had 34 children in care with 4 staff. The required ratio of 1 staff to 8 children was not being met. 2/22/19: 9:30am-3:15pm the preschool class (4-5 year olds) had 25 children with 2 staff. The required ratio of 1 staff to 10 children was not being met. 3/11/19: 7am-7:15am there were 16 children aged infant-3 years combined in care of 3 staff, with a required 1 staff to 4 children ratio that was not being met. 3/11/19: 7:15am-7:20am there were 14 three year old children in care with 1 staff. The required ratio of 1 staff to 10 children was not being met. 3/11/19: 8am-8:15am there were 5 infants in one classroom with 1 staff; and 6 infants in another classroom with 1 staff. The required 1 staff to 4 children ratio was not being met in either classroom. Also, in the preschool class (3-5year olds) there were 15 children with 1 staff. The required 1 staff to 10 children ratio was not being met. 3/11/19: 8:15am-8:30am there were 12 preschool aged children (4-5 years) in care with 1 staff. The required 1 staff to 10 children ratio was not being met. There were 31 three-four year old children in care with 3 staff. The required 1 staff to 10 children ratio was not being met. There were 19 two year olds in care with 2 staff. The required 1 staff to 8 children ratio was not being met. There were 14 toddlers in care with 2 staff. The required 1 staff to 5 children ratio was not being met. Staff stated during interviews that the morning ratios have been an issue for at least 2 months. Issues with ratio begin at approximately 7am and continue through at least 9 am most mornings each week. Plan of Correction: PD will work closely with RD and HR to continue to be fully staffed to meet and adjust schedules as needed to meet standards. 3/25/2019
Standard #: 22VAC40-185-510-A Description: Based on review of medications and supportive documents the facility failed to maintain compliance with this standard as required relating to authorization to administer medications. Evidence: The medications for children D and E did not have a current authorization to administer form for the on hand medications found in the medication box. The permission to administer for child D had expired as of 3/8/19. The permission to administer for Child E had expired as of 2/1/19. Both medications were in the active status for emergency administration. Plan of Correction: Child D is no longer on meds and Child E has a current form and permission on file. The PD will ensure that when children are on meds that he current form and permission will be current. 3/25/2019
Standard #: 22VAC40-185-560-F Description: Based on review of the facility and posted documents the facility failed to maintain compliance with this standard as required relating to posting of the menu. Non-compliance with this standard was previously cited on 11/26/18, and therefore is marked as a repeat violation. Evidence: In the hallway at the main entrance the menu posted was for the previous week of March 3rd-8th of 2019. Plan of Correction: The PD will ensure that he menu gets posted by Monday morning upon opening of the center. 3/25/2019
