We discussed the requirement for keeping children's medical records on site. The risk assessment for a violation was provided. We talked about playground maintenance. Also, we reviewed injury record requirements and documenting all staff present.
Thank you for your assistance during the unannounced renewal inspection conducted from 10:52 AM to 2:15 PM at the program site and from 2:45 PM to 4:00 PM at the agency office. Today, there were forty children, ages three to five, with six staff. I viewed daily routines, program activities, staffing, supervision, interactions with children, posted information, medication, emergency supplies and drill records, insurance, injury records, five records for children, agency inspections, lunch, menu, illness policies, six staff records, staff qualifications, staff training, indoor and outdoor areas. The children enjoyed active play outside. The teachers have planned a variety of learning experiences. The teachers were observed providing frequent interactions with children and responding to children's needs for guidance and care. Standard #: 22VAC40-185-60-A Description: Based on a review of five records for children, and interviews with administrative staff, the center failed to maintain and keep at the school site all required information for two children. Evidence: There were not copies of physical examination records in the files for child 1 and 2. The files at the program site only contained the immunization reports. Administrative staff contacted the main office and learned that the missing reports were in the main office files. Plan of Correction: Administrative staff immediately arranged for copies of the missing physical examination records to be scanned to staff at the school site. The records were printed, viewed by relevant staff, and filed. The assigned staff at the school will view all children's records to make sure they contain all required medical information on site. In the future, new children's records will be checked to ensure all information was obtained for the program site.
