Technical Assistance:
Suggestions were made for administrative staff to frequently visit classrooms for closer supervision of staff.

Comments:
An unannounced non-mandated inspection was conducted from 11:05am to 11:40am on 3/22/19. The inspection was conducted in response to a self reported incident on 3/20/19, related to staff found sleeping in the 2's classroom while supervising children. 1 staff's record and 1 child's record was reviewed. The Child Supervision Records for that date were reviewed. Staff and parent interviews were conducted. Based on the preponderance of evidence, non-compliances related to the incident have been cited in the Violation Notice. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on staff interviews and record review, staff supervising children did not always ensure care, protection and guidance. Evidence: 1. On 3/20/19, at approximately 8:00am Staff # 2 was found sleeping on a chair by Parent # 1. The Child Supervision Record indicated there were 5 two year old children present in the classroom present at the time. 2. Staff # 1 stated that between 8:05am to 8:10am, Parent # 1 (Child # 1's parent) called the center to inform her that upon entering the 2's classroom, Parent # 1 observed Staff # 1 sleeping. Staff # 1 was seen sleeping on a chair located to the the left side of the entrance door, next to the cubbies and large shelf. 3. Parent # 1 stated that Child # 1 and Parent # 1 entered the classroom, Staff # 2 was asleep on the chair with her head down on her chest. Parent # 1 stated when Parent # 1 got to Child # 1's cubby Staff # 2 woke up. 4. Staff # 3 stated that Staff # 2 informed her that Staff # 2 had a headache, and had taken an over the counter pain reliever and fell asleep. Staff # 3 stated that Staff # 2 informed her that she could not have been asleep for more than 3 minutes.

Plan of Correction: We have implemented a detailed corrective action plan based around supervision.

Standard #: 22VAC40-185-350-A
Description: Based on record review, the center did not maintain the required staff to children's ratio with children in care. Evidence: 1. On 3/20/19, in the two year old classroom, Staff #1 was the only staff member signed in with 9 two year old children, not meeting the ratio of 1 staff member for every 8 children. 2. The written documentation on the Child Supervision Record (CSR) for the two year old classroom indicated that Staff # 1 had 9 children between 8:07am to 8:16am. There were no additional staff members signed in during this time, leaving Staff # 1 over ratio for 9 minutes.

Plan of Correction: We will adjust schedules to ensure ratio requirements are met and retrain teachers to inform a supervisor when they are close to becoming over ratio.
