63.2(17) License & Registration Procedures

Comments:
An unannounced renewal inspection was conducted from 11:30am t0 2:45pm with the center director and staff. There were 77 children in care ranging in ages from three months to four years old. The children were observed during rest time and free play. Seven children's records were reviewed along with eight staff records. One medication was reviewed. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation not all areas of the playground were maintained in a safe condition. Evidence: on the play ground in front of the red slides there is a nail sticking out posing a tripping or puncher hazard.

Plan of Correction: Staff hammered the nail in.

Standard #: 22VAC40-185-330-B
Description: Based on observation the resilient surfacing in the fall zones was not six inches in depth. Evidence: The mulch under the two sing sets and both sets of slides was compressed and worn to the dirt base.

Plan of Correction: Mulch will be ordered this week.

Standard #: 22VAC40-185-510-J
Description: Based on observation not all mediation were stored in a locked place that would prevent access by children. Evidence: In the Farm room there was a bottle of vitamins stored in a unlocked drawer accessible to the children. The bottle did not have a person name on it. LI was told the bottle belonged to a staff member.

Plan of Correction: Removed during the inspection.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of eight staff files not all staff had fingerprinting checks done. Evidence: Staff A with a start date of 11/1/2008 and Staff B with start date 9/10/2017 did not have documentation of finger print checks.

Plan of Correction: We will complete the finger print checks and send them to your office.
