Comments:
An unannounced monitoring inspection was conducted today from 10:08am-2:00pm. There were 96 children (infants-5yrs.) directly supervised by 53 staff. The physical plant, 5 staff records, 9 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, story time, singing, dancing, coloring, being bottle fed, napping, practicing cutting and writing skills, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, it was determined that the center did not obtain a physical examination by or under the direction of a physician for each child enrolled. Evidence: Child#4's record is missing documentation of a physical examination completed by or under the direction of a physician.

Plan of Correction: We will ask the parent to submit a new physical form.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not have documentation that they had viewed the proof of identity of each child within seven days of the child's first day of attendance. Evidence: The records of child#1, child#6 and child#9 do not contain documentation that each child's proof of identity was verified within seven business days of their first day of attendance.

Plan of Correction: We will contact the parents and have them bring the documentation needed.

Standard #: 22VAC40-185-510-G
Description: Based on observation and interview, it was determined that two long term medications on-site did not have a medication authorization form completed by a physician. Evidence: Child#10 has a long term medications on site with an authorization form that was signed by the parent, but it was not completed and signed by the child's physician. Child#11 has a long term medication on-site without an medication authorization form completed and signed by the child's physician and the parent.

Plan of Correction: We will request an authorization form for each medication.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on staff record review, there is no documentation that a Child Abuse and Neglect check was initiated for an employee that has resided outside the state of Virginia within the last 5 years. Evidence: Staff#1 (DOH: 08/31/2015), has resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained.

Plan of Correction: We will locate the form and submit it to licensing.
