Comments:
An unannounced non-mandated inspection was conducted to investigate a self reported incident that occurred on 3/28/2019 regarding staff supervision. Staff interviews, a review of the center's own video recording system, review of staff records, written staff statements, parent interview and observation of the Beginner's Classroom and playground area were conducted on 04/01/19. A follow up inspection was conducted this date. Violations were found in the areas of administration and staff supervision. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on staff interviews, staff written statements, review of the center's policies and review of the center's own video recording system, the licensee failed to ensure compliance with the center's own policies and procedures. Evidence: l. The Center's Face to Name Policy requires that staff identify each child by face and name as each child crosses a threshold, staff are then required to change the child's location on an electronic device. Center policy requires staff to complete a final observation of the classroom prior to leaving the classroom. 2. On 03/28/19, the center's video recording system recorded Staff #1 failed to identify Child #1 (2yrs) by face as the class was exiting the classroom to go outside to the playground. 3. Staff #1 recorded on the electronic device that Child #1 had transitioned to the outside playground. 4. Staff #1 failed to complete a final observation of the classroom to ensure that no children remained in the room, leaving Child #1(2yrs) alone in the classroom for approximately 5 1/2 minutes.

Plan of Correction: Every playground transition will be observed by management and management will document the transition on a transition log daily. The logs will be available to the licensing inspector. for review.

Standard #: 22VAC40-185-340-F
Description: Based on staff interviews, staff written statements and the center's video recording system, one child was not within actual sight and sound supervision of staff. Evidence: On 03/38/19, Child #1 (2yrs)was left alone in the Beginner's Class for approximately 5 1/2 minutes, while the the child's class transitioned outside to the playground. Staff #1 and Staff #2 were unaware that the child was not with the remainder of the class until Staff #3 found Child #1 alone in the classroom.

Plan of Correction: Both staff were terminated. Center will add additional physical equipment to assist with sight supervision, management to complete documentation of transition logs and in service staff regarding overall supervision compliance. Ongoing.
