An unannounced monitoring inspection was conducted from 10:30 a.m. - 12:10 p.m. There were 35 children in care being supervised by 7 staff members. The children were observed participating in outdoor play and structured center play. Lunch is provided from home. The fall zones and use zones were measured on the large playground equipment, specifically the monkey bars, slides, swings, and the hexagon climbing structure. Standard #: 22VAC40-185-270-A Description: Based on observation of the large playground it was determined that not all areas were maintained in safe condition. Evidence: Three rope climbing apparatus were observed that were not attached to the ground at the bottom posing an entanglement hazard. Plan of Correction: The play equipment will not be used until the rope climbing apparatus can be attached to the ground.
Standard #: 22VAC40-185-330-B Description: Based on observation of the large playground it was determined that the appropriate resilient surfacing and use zones for play equipment were not met. Evidence: During the inspection the LI (Licensing Inspector) measured the mulch in fall zones and use zone areas of the playground equipment. 1. Mulch was sporadic and dirt was visible under the monkey bars where mulch measured less than 1 inch in fall zone areas. 2. Mulch in use zones of the slides measured 4'3" on the right and 4'5" on the left. 3. Mulch in use zones of the monkey bars measured 3'10" in the front and 4'4" in the back. A six foot fall zone is required around equipment to anticipate the trajectory in the event of a fall while the equipment is in use. 4. A tree stump was observed in the fall zone directly next to the hexagon climbing apparatus. Plan of Correction: The playground equipment will not be used until the appropriate fall zones and use zones are met.
Standard #: 22VAC40-185-340-F Description: Based on observation of the primary class caring for children ages 3 - 6 years old it was determined that staff did not always maintain sight and sound supervision of children under 10 years of age. Evidence: The LI (Licensing Inspector) observed two staff members sitting on the carpet with 19 children while 3 children were observed outside sweeping the sidewalk out of sight and sound supervision. Plan of Correction: Staff will maintain sight and sound supervision of children in care.
Standard #: 22VAC40-185-500-B Description: Based on observation of the toddler room it was determined that not all diapering procedures were followed. Evidence: Child A was observed being diapered on a wooden bench and not on the changing table as required. The bench was not cleaned and sanitized after diapering. Plan of Correction: Staff will be retrained on diapering procedures.
