Comments:
An unannounced monitoring inspection was conducted on June 3 2019 between 10:00am and 12:30pm. There were 6 children in the direct care of 2 providers. The children were observed playing in the large gym space and working on art projects. The center is clean and organized. There are age-appropriate materials for the children to utilize. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, 1 of 5 children's records reviewed did not have documentation that the child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child #1 record did not contain an immunization documentation

Plan of Correction: I will make a copy of the records and have them at both centers.

Standard #: 22VAC40-185-510-G
Description: Based on observation, the center did not administer prescription medication from the original container. Evidence: Staff stated that a parent refilled the medication bottle that is located in the center.

Plan of Correction: I will have the parents bring the new bottle and switch it with the old bottle.
