An unannounced complaint inspection was conducted in response to complaint regarding supervision received by the department on April 11, 2019. The inspection was initiated and completed on May 14, 2019. The inspector was on site from 10:30 am until 4:00 pm. Interviews with staff were conducted, observations were made and documentation was reviewed. The preponderance of evidence gathered during the investigation supports the allegation and therefore the complaint is determined to be valid. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-185-350-E-1 Complaint related: Yes Description: Based on interview and review of documentation, the center did not maintain a ratio of one staff for every four children for children from the age of birth to the age of 16 months. Evidence: 1. Staff stated on April 8, 2019, children present in the center were combined in one group from approximately 9am until 10:30 am being supervised by two staff. 2. The daily attendance record, dated April 8, 2019, indicated there were twelve children present and in care during the time period of 9:00 am until 10:30 am. 3.
