A Subsidy Health and Safety inspection was conducted on 7/29/2019 to review supplemental health and safety requirements. The inspector arrived at 8:30 and departed the program at 1:15 pm. There were 22 children with five staff present. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-540-A
Description: Based on review of five staff's records, the center failed to ensure that all TB screenings were completed within the last 30 days prior to employment. Evidence: 1. Staff # 4 was hired on 7/16/2019 and on the day of the inspection did not have a TB screening or test. 2. Staff # 5 was hired on 7/8/2019 and on the day of the inspection did not have a TB screening or test.

Plan of Correction: The director will send the staff member ASAP to obtain a TB screening/test. In future all new hires will have a TB prior to starting.
