63.2 Child Abuse & Neglect
An unannounced monitoring inspection was conducted with the director and owner beginning at 11:22 AM and ending at 2:10 PM. There were twenty-three children, ages two through five years old, in the care during the inspection. The children were observed playing during free play, dancing to music, and playing outside on the playground. The children were also observed eating lunch brought from home. Ten children's records were reviewed. Four staff records were reviewed. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The emergency evacuation practice drill log was reviewed and in compliance. The most recent health inspection was dated 12/03/2018. The most recent fire inspection was dated 8/30/2018.

Standard #: 22VAC40-185-130-A Description: Based on a review of ten children's records, the center failed to obtain documentation of immunizations for all children enrolled prior to the first day of attendance. Evidence: Child A's(start date: 9/3/19) record was missing the documentation of immunizations. Plan of Correction: The center has requested the information from the parent and are waiting for the parent to provide the documentation. The center director will remind the parent that they need the missing documentation.
Standard #: 22VAC40-185-60-A Description: Based on a review of ten children's records, not all of the children's records were complete. Evidence: Child A, child B, child C, child D, child E, child F, child G, child H, child I, child J's records were missing the address and phone number for the parent's place of employment. Plan of Correction: There was no section for the contact information of the parent's place of employment on the 2019-2020 registration form. The registration form will be updated and provided to parents to obtain the missing information.
Standard #: 22VAC40-185-70-A Description: Based on the review of four staff records, not all of the staff records were complete. Evidence: Staff A and staff B were missing documentation of education, orientation training, staff development, certification, and experience required by the job position. Plan of Correction: The center will review the qualifications and obtain the missing documentation for the staff records.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to keep all hazardous substances in a locked place using a safe locking method that prevents access by children. Evidence: In the multipurpose room, there was an unlocked cabinet with a spray bottle of cleaning solution, Lysol disinfectant, dish washing liquid, 2 boxes of Alka Seltzer, and 3 boxes of Borax.In the two's classroom, a staff's purse was stored underneath a counter and had a lighter inside of it, the area was accessible to children. Plan of Correction: The center will ensure that all cabinets with hazardous substances will be locked and inaccessible to children. Staff will receive retraining regarding the storage for their personal items.
