Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:00 am - 11:00 am. There were twelve preschool children in care with two staff. The children were observed in group activities, playing in the centers and at circle time. Five children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the facility in a safe and operable condition.
Evidence:
1. The inspector observed the outside play area. There are three holes in the ground next to the basketball court. The holes are large enough for a child's foot and could cause a child to fall or trip. Also on the play area, there are two landscape timbers surrounding the swings that are split with sharp edges and the grass is high. In the classroom, the inspector observed a handle on a base cabinet under the child's sink that has come loose and in the bathroom on the right the paper towel holder is loose from the wall.

Plan of Correction: The teacher will complete a maintenance request to have the repairs made. The children will not play in the area of the playground with the holes in the ground until the holes are filled.
