Technical Assistance:
Discussed playground surfacing for the swings and ratios.

An unannounced monitoring inspection was conducted on September 4, 2019 from approximately 9:35 am-12:30 pm. Upon arrival to the facility, there were 40 children with nine staff members. Additional staff were on site for administrative and support purposes. Children were observed doing music and movement, playing outside, science activities, station activities and during storytime. Licensing Inspector reviewed supervision, activities, equipment, indoor areas, diapering and handwashing procedures, medication, children and staff records, emergency evacuation and shelter-in-place drill. Five children's records and Five staff records were reviewed. The outdoor playground was inspected.

Standard #: 22VAC40-185-60-A
Description: Based on record review and staff interview, in one of five children's records reviewed, the licensee did not ensure that each enrolled child's record contained all information to meet compliance.

Evidence: In the record for child #1 there was no documentation of the parent's work phone, physician's name and phone number. Staff #6 verified the record was missing documents.

Plan of Correction: Child was enrolled on the day of inspection and proper documents were provided by the guardian at the end of the day on 9/4/2019. Parents cell phone # was verified as their work #

Standard #: 22VAC40-185-340-G
Description: Based on observation the licensee did not ensure children 10 years of age are older are within actual sight and sound supervision of staff unless staff can hear or see child, can provide immediate intervention and persons other than staff are not allowed in the area.

Evidence: Upon arrival Child # 1( age 11) and child #5 (age 12) were observed alone in the lobby area . Staff were walking assisting other staff in the building. Staff need to be able to provide immediate intervention.

Plan of Correction: While supervisor steps away fro the lobby will either accompany her or will be placed in a classroom (within ratio)

Standard #: 22VAC40-185-440-L
Description: Based on observation, the center did not ensure that pillows were not used by children under two years of age.

Evidence: Child #1(10 month old) infant was laying in a crib propped up on a "Boppy" pillow .

Plan of Correction: Boppy pillow was removed immediatley by supervisor and informed staff this is not allowed.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the licensee did not ensure staff wash their hands with soap and running water before and after helping a child use the toilet or a diaper change.

Evidence: Staff did not wash hands immediately after diapering a child in the Wood Chuck classroom. Staff opened cabinet and gate before staff washed her hands.

Plan of Correction: Remind staff member of correct hand washing procedures and the consequences of not following policy

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interview, the licensee did not ensure the diapering surface shall be used only for diapering or cleaning children, and cleaned with soap and at least room temperature water and sanitized after each use.


A child's diaper was observed being changed in the Wood Chucks classroom, the staff member sprayed the surface with a "disinfectant" spray but did not clean the surface with soap and water or a different cleaning agent. The clipboard was on the diapering surface in the Nursery.

Plan of Correction: Reminded staff member of correct diapering surface sanitation. Remind staff member not to place anything on diapering surface.

Standard #: 22VAC40-185-550-M
Description: Based on 2 of 2 injury reports reviewed, the licensee did not ensure required information is documented in written record of minor injuries.

Evidence: Two injury report documents from September 3, 2019 did not have how parents were notified. Reviewed with staff and confirmed missing documentation.

Plan of Correction: Discard all old copies of injury reports. Handed out correct copies that provide how parents are notified.

Standard #: 22VAC40-185-570-A
Description: Based on observation and staff interview, the licensee did not ensure when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.

The activity table with bucket seats used when children are fed did not have protective belts. Staff in the nursery room stated she does not have belts for the bucket seats and they are used during feeding time.

Plan of Correction: Protective belts were placed on each bucket seat by supervisor and instructed nursery staff they must be used at all times.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of board officer records, the facility did not obtain a fingerprint based background check within the required time frame.

Evidence: The center did not have a the fingerprint based background check determination letter for board officer #1.

Plan of Correction: Fingerprint based background check was conducted on 9/11/2019 and received.
