Comments:
A subsidy health and safety inspection was completed on September 6, 2019 from approximately 12:15 p.m. to 3:15 p.m. to review supplemental health and safety requirements. Twenty-one children were in care and six staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, reading, singing, preparing for nap time, departure from the center, and nap time. Interviews and interactions were conducted and no medications are given. Five children?s records, five staff records and three corporate officer records were reviewed. The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on a review of staff records, the licensee failed to have staff evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB), submitted at the time of employment and prior to coming into contact and completed within the last 30 calendar days of the date of employment for one of five staff.
Evidence: 1. Staff #2's record, hired on August 5, 2019, contained a tuberculosis screening/test dated March 11, 2019 and therefore not completed within the required time frame. 2. Administrator #1 acknowledged that this tuberculosis screening/test was not obtained within the required time frame.

Plan of Correction: This cannot be corrected.
