Technical Assistance:
Reviewed proper documentation of each staff person's date of employment.

Reviewed class requirements for Recognizing Abuse and Neglect training.

Comments:
An unannounced monitoring inspection was conducted today. There were 81 children in care, with a total of 13 staff members assigned to their direct supervision. Children's records, staff training, background checks, staff records, the indoor/outdoor physical plant, classroom programming, and children's medications were inspected today.

The children were observed actively engaged in a wide range of age-appropriate activities, including selecting pumpkins from the on-site pumpkin patch, fingerpainting, enjoying creative play activities, decorating pumpkins, enjoying circle time discussions, and playing on the playground.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that 5 records do not contain all required information.
Evidence:
Child records A, B, C, D and E do not include each parent's work phone number.

Plan of Correction: Parents complete the forms electronically, and can tab paste the work phone number if not applicable. We will update our electronic registration form so that parents cannot tab past this section without entering the required information.

Standard #: 22VAC40-185-70-A
Description: Based on a review of new staff records, it was determined that one record does not include all required information.
Evidence:
Staff record D does not include a second reference check that meets all specified criteria.

Plan of Correction: Another reference check will be completed.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that one potentially hazardous substance was not kept in a locked place.
Evidence:
In the Pups classroom, one container of table cleaning wipes was stored inside an unlocked upper level cabinet.

Plan of Correction: That cabinet is not used by the Preschool. It is only used by the Sunday school program. The cabinet will be locked and staff will make sure to check the cabinet every Monday morning before children arrive.
