Child #2's action plan list an over the counter antihistamine that is not on-site.
An unannounced monitoring visit was conducted today. 77 children were in care with direct care staff in ratio for supervision. Observed circle time, clay and free play, transition to the outside playground area, diapering and infant care. Emergency preparation is current and medications were on-site for emergency purposes. Lunch served today was chicken tenders, broccoli, corn bread and oranges. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-130-B Description: Based on record review, 1 of 3 children's records needs an update immunization record.
Child #1 is under two years of age and the current immunization record is older than 6 months (1/2019)
Plan of Correction: Will request an updated immunization record from the parent.
Standard #: 22VAC40-185-270-A Description: Based on observation, an area of the center is not being maintained in an operable condition.
One of the toilets in the girls hallway bathroom is not working.
Plan of Correction: A maintenance request has been submitted to repair the toilet.
Standard #: 22VAC40-185-370 Description: Based on observation and staff statements, there is not place to document "tummy time" for infants on the daily report.
Staff stated that the new electronic program does not have a place to document 30 minutes of tummy time each day for the infants who can not roll over on their own.
Plan of Correction: Tummy time will be documented on the electronic daily sheet each day to verify the required 30 minutes.
