A subsidy health and safety inspection was completed on November 26, 2019 from approximately 11:15 a.m. to 1:45 p.m. to review supplemental health and safety requirements. Twenty children were in care and six staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include transitioning to lunch, lunch time and nap time. Interviews and interactions were conducted and medications were reviewed. Seven children?s records and six staff records were reviewed. The owner/director and an administrative staff were available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.
Standard #: 22VAC40-665-540-A Description: Based on a review of staff records, the licensee failed to have staff evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB), submitted at the time of employment and prior to coming into contact and completed within the last 30 calendar days of the date of employment for one of six staff.
Evidence: 1. Staff #1's record, hired on September 3, 2019, contained a tuberculosis screening/test dated January 15, 2019 and therefore not completed within the required time frame. 2. Administrator #1 acknowledged this tuberculosis screenings/test was not done timely but indicates he was not aware of this policy change.
Plan of Correction: The owner will have staff get the tuberculosis test/screening at hire or will accept results that are within 30 days per the requirements.
