Comments:
An unannounced monitoring inspection was completed on November 26, 2019 from approximately 11:15 a.m. to 1:45 p.m. Twenty children were in care and six staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include transitioning to lunch, lunch time and nap time. Interviews and interactions were conducted and medications were reviewed. Seven children?s records and six staff records were reviewed. The owner/director and an administrative staff were available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for two of six staff.
Evidence: 1. Staff #1's record, hired on September 3, 2019 as a program leader, lacked documentation of the training/education for the job description. In an interview, Staff #1 reported she has taken enough classes to qualify her as a program leader but she acknowledged she has not submitted documentation of this. Staff #4's record, hired on September 5, 2019 as an assistant, lacked documentation of initial orientation training. In an interview, Administrator #1 stated he completed the initial training with this staff but acknowledged this is not documented in the record.

Plan of Correction: The owner will obtain this missing training documentation for these records.

Standard #: 22VAC40-185-240-C
Description: Based on a review of staff records, the licensee failed to ensure staff obtained 16 clock hours of annual training for three of six staff.
Evidence: 1. Staff #2's record reflected approximately eight hours of annual training, Staff #5's record reflected approximately eleven hours of annual training, and Staff #6's record reflected no annual training for each of their previous training years. Administrator #1 acknowledged these staff lacked the required amount of annual training.

Plan of Correction: The owner will create a spreadsheet to track each staff's annual training.

Standard #: 22VAC40-185-540-A
Description: Based on observation, the licensee failed to maintain a first aid kit wherever children are in care.
Evidence: 1. Administrator #1 was observed leaving the center with two children in a vehicle. Upon his return, the vehicle was observed with no first aid kit present. 2. In a interview with Administrator #1, he reported that he took these two children home today in his vehicle but that Administrator #2 usually transports them in the center's van, which has the first aid supplies.

Plan of Correction: The owner will not use his vehicle again to transport children.

Standard #: 22VAC40-185-580-C
Description: Based on observation and interview, the licensee failed to ensure that emergency numbers, the centers'name, address and phone number, and a list of the names of the children being transported were in a transportation vehicle.
Evidence: 1. Administrator #1 was observed leaving the center with two children in a vehicle. Upon his return, the vehicle was observed with none of the required items for transportation. 2. In a interview with Administrator #1, he reported that he took these two children home today in his vehicle but that Administrator #2 usually transports them in the center's van, which contains all the required items.

Plan of Correction: The owner will not use his vehicle again to transport children.
