Comments:
An unannounced monitoring inspection was conducted today from approximately 8:50am-10:30am and on 11/10/2019 from approximately 11am-1:30pm. There were 110 children in the direct care and supervision of 30 staff. A sample of 10 children?s records and 5 staff records were reviewed. The children were observed eating breakfast, choosing helpers, singing, brushing their teeth, washing their hands, learning about 5 senses, and engaged with staff. Two new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the Human Resources. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, each child did not have an immunization on file.

Evidence:
Child C did not have an immunization record on file.

Plan of Correction: I will request that from the parent today.

Standard #: 22VAC40-185-60-A
Description: Based on observation, the record for a child in care was incomplete.

Evidence:
1. Child A was missing the addresses for two emergency contacts.
2. Child B was missing the employer and employer phone number of both parents.

Plan of Correction: I will request the missing information from the parents today.

Standard #: 22VAC40-185-290-3
Description: Based on observation, electrical outlets did not have protective covers.

Evidence:
1. There were 15 uncovered outlets in the gym.
2. There were 10 uncovered outlets in the bathroom connected to the gym.
3. The Candlewick Class, The Harmony Class and The Confetti class were in the gym playing.

Plan of Correction: We will cover them all with protective covers.

Standard #: 22VAC40-185-510-D
Description: Based on interview and observation, the authorization for medication was not available to staff during the effective period.

Evidence:
1. The medication authorization was not available for staff
2. Staff A stated some authorizations were on the computer of Staff B

Plan of Correction: I have them and will put them in the binder.

Standard #: 22VAC40-185-510-E
Description: Based on observation, medication was not properly labeled.

Evidence:
The medication of Child D was missing the physicians prescription label, name on box, and dosage instructions.

Plan of Correction: I copied the information and discarded the paperwork, I will keep it for next time.
