Comments:
An unannounced monitoring inspection was conducted today from 10:30am through 1:00pm. There were nine children in care ages two through four years old with two teachers. Children and staff records were reviewed. Required postings, annual inspections, emergency evacuation drills and first aid kit were reviewed. The program director reports that there is not any medication being administered nor are children in care transported. Children were observed in teacher and self-directed activities including restroom routines, transitions hand washing, lunch and learning play activities. There were not any violations identified during this inspection. Violations regarding physical plant, emergency items and sanitation were identified and appear on the violation notice. The violations were discussed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure that areas are maintained in safe operable condition.
Evidence:
1 - There are two classrooms that are stacked with equipment and materials. The rooms are stacked in such a way to pose a potential risk to children in care. Children were observed entering each of the rooms during the inspection. The items stacked and stored in the room pose toppling and tripping hazards.
a. The rooms are not locked and have no child proof devices on the doors. Therefore, the contents of the room are accessible.
b - Staff 1 stated that the materials had been stacked in the rooms in order to prepare the larger room for the center Christmas party.
3 - On the counter top and in the cabinets, in the large child care room, there are choking hazards accessible to children including but not limited to beads, paper clips and AAA batteries.
a. Staff 1 confirmed that the batteries were accessible on the counter and the cabinets were not locked.

Standard #: 22VAC40-185-340-F
Description: Based on observation the center failed to demonstrate that children under 10 years of age always are within actual sight and sound supervision of staff.
Evidence:
1 - For approximately 1 minute, two children were left in the classroom, alone and unattended by any staff. Both staff 1 and 2 were in the rest room with seven children.
2 - Staff 1 confirmed that the children had been left unattended when staff 2 walked out of the classroom, leaving two children unattended.

Description: Based on observation of equipment and interview the center failed to demonstrate compliance with having one working, battery-operated radio.
Evidence:
1 - There was not a working battery operated radio available during today's inspection.
2 - Staff 1 confirmed that there was not a battery operated radio available today.

Standard #: 22VAC40-185-560-K
Description: Based on observation the center failed to demonstrate tables are sanitized before each use for feeding.
Evidence:
1 - The children had completed work tasks and transitioned from using the tables for those tasks to eating lunch on the same tables. The tables were not sanitized before the children were allowed to eat on the same tables.
2 - Staff 2 stated that she used water to wipe the table with water only prior to children eating lunch on the tables.
