63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted on January 24 2020 between 9:15 am and 12:45pm. There were 80 children in the direct care of 13 staff members. Children were observed eating lunch (macaroni and cheese, peas, and milk), playing on the playground, washing hands, and preparing for nap time. The center is clean and organized. Classrooms maintained ratios throughout the inspection. Areas of non-compliance are listed on the violation notice. Standard #: 22VAC40-185-160-A Description: Based on record review, 2 of 6 staffing records did not have the required documentation regarding a Tuberculosis screening.
Evidence: Staff #4 record has a digital document on file, but was not able to be opened while the inspector was on site. Staff # 6 record did not have documentation that a tuberculosis screening was submitted.
Plan of Correction: We are going to send staff to get their TB test completed and will request a duplicate document from the employee that's digital file could not be opened.
Standard #: 22VAC40-185-560-G Description: Based on direct observation, the center did not ensure food brought from home was labeled with a child's name and dated.
Evidence: Food the Bubble Buddies 2 classroom was labeled with children's names, but not dated for the date that it was brought into the center.
Plan of Correction: We will put masking tape and a marker in the classroom to being dating food brought from home.
Standard #: 22VAC40-191-40-D-1-B Description: Based on record review, 1 of 6 staffing records reviewed did not have documentation that the center submitted and received a completed central registry background search prior to the end of the first 30 days of employment.
Evidence: Staff # 3 record does not have documentation that the center submitted a central registry search.
Plan of Correction: We will send out the document today.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, 3 of 6 staffing records reviewed did not have documentation that a fingerprint check was completed.
Evidence: Staff #2, #3, and #5 records do not have documentation that states that the employee had a completed fingerprint check prior to first date of employment. Staff #
Plan of Correction: We will have staff make appointments and go today to get it completed.
