Comments:
Today an unannounced Monitoring Inspection took place between the hours of 10:15am and 12:45pm. There were 2 classrooms observed with a total of 15 children with 8 staff within the supervision guidelines. Upon arrival, the children and staff were observed practicing a fire drill in the program's parking lot. Inside, infant care was observed. Preschoolers were observed playing on the playground, singing, eating lunch (beef picadillo, salad, pears and milk) and napping. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #2 (DOB: 3/29/2018) most recent immunizations are dated 2/1/2019.

Plan of Correction: I'll talk to the family advocate to see if whether the parents have provided the document.
