Comments:
An unannounced monitoring inspection was conducted on February 10, 2020. There were 22 children present, the children were observed in two classrooms. Each group was supervised by two staff members. The ages of the children ranged from 3 to 5 years. The inspector arrived at 9:00 a.m. and departed the program at 11:45 a.m.
The children were observed during free play, outside time,music, story time and circle time and lunch.
A sample of 5 children's records was reviewed. Documentation of the fire and health inspections was reviewed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were complete.

Evidence:
1. Child # 2 was missing the contact phone number for the physician.
2. Child # 5 was missing previous child care and chronic physical problems and allergies.

Plan of Correction: The FSW will obtain the missing information and will monitor all children's records.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all chemicals were locked in a way that prevents entry.

Evidence:
1. Classroom A locking method did prevent entry into the cabinet where all of the chemicals are stored. The LI could place her arm in between the doors and pull out chemicals.

Plan of Correction: The site director will contact maintenance to have the a lock or new doors added to the cabinet.
