Technical Assistance:
Today we discussed that the children in care are using the playground equipment this summer, volunteers, TB tests and a 2-step process to clean and sanitize tables.

An unannounced monitoring inspection of the facility and records was conducted from 9am - 1:30pm. There were 90 children in care, ages five to 12 with 23 staff members. Interviews were conducted with children and staff present. Five children?s records and seven staff records were reviewed. Children were observed at breakfast, transitions, leaving for the center to go to swimming at the YMCA, hand washing, rest room routines, music, teacher and self-directed activities. The inspector reviewed the center first aid kit, emergency radio and flashlight, medication as well as current insurance documentation, fire and health inspection and emergency evacuation drills. Violations were found in the following areas and appear as violations on the violation notice: peeling paint, unlocked chemicals, documentation of annual training hours, and medication administration.

Description: Based on observation and interview the center failed to ensure that staff records contain all required information. Evidence: 1 - The record for staff 1 contains documentation of 4 and 3/4 hours of training for the 2014 - 2015 year where documentation for 10 hours required. 2 - Staff 2 and 3 confirm that documentation was not present.

Plan of Correction: Center management will obtain the required documentation from the staff.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure areas shall be maintained in safe condition. Evidence: 1 - There are areas of peeling paint, within reach of children on the outside doors leading from building 1 to building 2. 2 - Staff 1 confirmed that the there are areas of peeling paint.

Plan of Correction: Center management will place a work order with the school to have the area re-painted.

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview the center failed to ensure that hazardous substances shall be kept in a locked place. Evidence: 1 - In the room used for art classes for the children in care, in a cabinet over the sink, an unlocked location, are two pump bottles of hand sanitizer and one canister of "comet" bathroom cleaner. The hand sanitizer label reads 67% ethal alcohol. The comet reads caution, eye irritation. 2 - Staff 1 confirmed that the items were located in an unlocked location.

Plan of Correction: Corrected during inspection. The items were removed from the cabinet and a pad lock requiring a key was placed on the cabinet.

Standard #: 22VAC40-185-510-G
Description: Based on record review and interview the center failed to ensure that the center administers drugs only to the child identified on the prescription label in accordance with the prescriber's instructions pertaining frequency. Evidence: Albuterol was administered to two different children on different days, one on 6/29/15 and another on 7/8/15. However, the written authorization obtained to administer these medications state "as needed." The directions related to frequency do not indicate any additional information that would define when "as needed" is applicable.

Plan of Correction: Center management will ensure that complete information regarding when to administer medications is obtained from the person who leaves the medication at the center.
