An announced renewal inspection was conducted from 10am through 1pm. There were 104 children present for care. Children and staff records were reviewed. A walking tour of the two campus buildings and outside grounds was completed. Required postings and annual inspections were reviewed. Children were observed in teacher and self-directed activities including reading, lunch and transitions going to and coming from swimming. Violations were observed in the areas of physical plant, training hours and required postings.
Standard #: 22VAC40-185-240-C Description: Based on record review and interview the center failed to ensure that each staff has documentation of 10 hours of annual training. Evidence: 1 - The record for staff 2 contains documentation of 1.5 hours of training where documentation of 10 hours is required. 2 - Staff 1 confirmed that the record contained documentation of less than the required 10 hours of training. Plan of Correction: Center management will obtain the required documentation from the staff.
Standard #: 22VAC40-185-270-A Description: Based on observation and interview the center failed to ensure that equipment is maintained in safe condition. Evidence: 1 - There is a cracked, hard plastic tote box in the first grade classroom. The box is on a shelf and within reach of children in care. 2 - Staff 1 confirmed that cracked tote box was within reach of children. 3 - There is unlabeled pink spray bottle sitting on a student desk in the second grade classroom. 4 - Staff 1 confirmed that the unlabeled spray bottle was within reach of children. Plan of Correction: The tote was removed during the inspection. Center management will remind and retrain staff in the importance to remove broken equipment from reach of children. The spray bottle was removed during the inspection. Center management will remind and retrain staff in the importance of labeling all spray bottles.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation and interview the center failed to ensure that the findings of the most recent inspection are posted in center. Evidence: 1 - The most recent violation notice posted in the center is dated 7/7/14. However, the most recent inspection was conducted on 7/14/15. 2 - Staff 1 confirmed that the posted inspection results are not the most recent results. Plan of Correction: Center management will ensure that most inspection results are posted in the future. Going forward if the results are not received through email, center management will download the most recent results from the department web site.
