63.2 Licensure and Registration Procedures

Technical Assistance:
Discussed with Staff the importance of making sure that the attendance record reflects the children present in the fire drill log.

Comments:
Today an unannounced renewal visit was conducted with Center Staff. This visit took place in the morning between 10:45am-1pm there were 13 children with 1 staff upon arrival. The children were observed having snack, arts and craft and playing free play games in the gym with staff. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-350-E-4
Description: Based on review of written attendance record, review of four children's records, staff interviews, and observation, the center did not maintain the required supervision ratio of staff to children. Evidence: Upon arrival at 10:45am, there were 13 children with one staff, however the youngest child present today was 3 years old. Staff #1 was in the gym, teaching a class.

Plan of Correction: Center staff stated that an extra staff person was suppose to arrive however they did not show up.

Standard #: 63.2-1720.1-B-3
Description: Based on interview with Staff and review of staff records, the center did not obtain a Out-of-state Central Registry check for staff. Evidence: 1. Staff #1's Sworn Disclosure Statement confirmed that they have lived out of the state. 2. Record for Staff#1 (hired 10/2017) did not have documentation of an out-of-state Central Registry check completed prior to employment. 3. Center staff confirmed that an out-of-state central registry check was mailed on June 15, 2018 for staff #1.

Plan of Correction: Staff stated that they submitted the form however, they will call to find out the status.
