An unannounced renewal inspection was initiated today at 1pm. The Provider was not present during the inspection, due to attending a training in the Fairfax licensing office. During the inspection, there were 8 children, totaling 19 points, under the supervision of 4 caregivers. The children were observed taking naps, playing quietly, being fed and interacting with the caregivers in a positive, constructive manner. The Provider is not currently MAT trained and there are no children with any medication needs during program hours at this time. One of the caregivers has begun taking the first phase to obtain a MAT certification. Should you have any questions, please contact me.

Description: Based on file review, the Provider failed to obtain documentation of a physical examination within 30 days after the first day of attendance. Evidence 1. Child #3 began receiving care on 9/8/14. There is no physical examination on file. 2. Child #7 began receiving care on 8/4/14. There is no physical examination on file.

Plan of Correction: The Provider has repeatedly asked the parents to bring this into the FDH. She will remind them of this.
