Comments:
Arrived 9:15am Departed 10:35am An unannounced monitoring inspection was completed today. Six children were in care with two caregivers and 17 points. The children were playing in the play room with cars and toys and eating a snack. Non compliances from the last inspection, seven children's records and two staff records were reviewed. An exit interview was completed at the conclusion of the inspection. Thank you for your cooperation. Please contact me if I may be of service.

Standard #: 22VAC40-111-100-A
Description: Based on the review of seven children's files, the facility did not obtain a health exam record for all children in care. Evidence: 1. There is no physical health exam record on file for child # 5 and 7. ( enrolled 11/11/14, 10/1/14)

Plan of Correction: The provider has requested this information from the parents. She will request the information again.

Standard #: 22VAC40-111-90-A
Description: Based on the review of seven children's files, the facility did not obtain an immunization record for all children in care. Evidence: 1. There is no immunization record on file for child #7. ( enrolled 10/1/14)

Standard #: 22VAC40-111-240-A
Description: Based on observation the facility did not maintain a safe environment in the playroom. Evidence: 1. There is an older model (non-flat screen) 19 inch television on a table (18-20 inches high) in the play area. It is heavy and is accessible to children.

Plan of Correction: The provider will remove the table and place the TV on the floor.
