Comments:
Today an unannounced monitoring visit was conducted with the Provider-Mrs. Batavia. This visit took place in the morning, there were 11 children present with the Provider and 2 assistants for a total of 30 points within the supervision guidelines. The children were observed having morning free play, lunch and in transition for nap time. Lunch today consisted of a thanksgiving potluck provided by the parents (chicken, macaroni and cheese, carrots, rolls, and mashed potatoes. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-720-A
Description: Based on observation of the physical plant, a nonprescription medication was found not labeled and not in original box. Evidence: Upon arrival, a nonprescription medication for a child was found on a table and not labeled with the child's name and not in original box.

Plan of Correction: Provider stated that a parent bought the medication today however she does not administer medication. Medication will be returned to the parent today.

Standard #: 22VAC40-111-730-D
Description: Based on observation of the physical plant, a medication was found not locked. Evidence: A nonprescription medication was found on a table unlocked and accessible to children.

Plan of Correction: Provider stated that a parent bought the medication today however she does not administer medication. Medication will be returned to the parent today.

Standard #: 22VAC40-111-960-E
Description: Based on observation of the bottles, one bottle was found not labeled with the child's name and date. Evidence: one infant bottle was found without the date.

Plan of Correction: Provider added the date during this inspection.

Standard #: 22VAC40-111-60-B
Description: Based on observation of the children records, one record did not include the parents work address . Evidence; Child #11's record did not include the parents complete work address.

Plan of Correction: Provider will call the parent today and request the information.

Standard #: 22VAC40-111-180-A
Description: Evidence - 1. On the date of inspection (05/06/2014), TB documentation was not available for: Member #1, and Member #4. 2. The following were the most current dates of the documentation provided: for Member #1 (05/05/2012); and for Member #4 (10/07/2011).

Plan of Correction: Provider stated that due to personal issues one of the members currently are not able to obtain a Tb screening. She will have her doctor provide a note.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the physical plant, one outlet was missing a protective cover. Evidence: One electrical outlet in the nap room was found missing a protective cover.

Plan of Correction: Provider corrected during this inspection.

Standard #: 22VAC40-111-540-A
Description: Based on observation of the children, 6 children were observed in high chairs without safety straps securely fastened. Evidence: Upon arrival, 6 children were observed in high chairs with the safety strap not securely fastened.

Plan of Correction: Staff strapped all of the children in the high chairs during this inspection.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review, 1 of 4 staff/household members' files reviewed indicated that it has been more than three years since the date of the most recent central registry finding (CRC). Evidence - 1. On the date of inspection (05/06/2014), a current (within the last 3 years) CRC finding was not available for Member #1. 2. The most recent CRC finding available for Member #1 was dated - 08/28/2009.

Plan of Correction: Provider stated that she is unable to locate the most recent background check however she will provide a copy and or have it updated.
