54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted from 10:45am - 12:00pm. There was one child age two in care with the provider for a total of two points. Free play was observed in the family day home. Lunch served today is corn dog nuggets, green beans, corn, bread, and milk. The last fire drill was conducted 11/18/2014. One children's record was reviewed and the provider record was reviewed. The first aid kit, weather band radio, and flashlight was reviewed and in compliance. The provider does not administer medication in the family day home. Standard #: 22VAC40-111-60-A Description: Based on review of 1 children's record it was determined that not all records were complete. Evidence: Child A with a start date of 8/25/2014 was missing the addresses required for two emergency contacts. Plan of Correction: The provider will request the emergency contact addresses.
Standard #: 22VAC40-111-210-A Description: Based on record review of the provider it was determined that the annual training required was not complete. Evidence: The provider had 2 of the required 16 hours of annual training on record. Plan of Correction: The provider will attend trainings to achieve 16 hours.
Standard #: 22VAC40-111-280 Description: Based on review of the family day home it was determined that cleaning agents were accessible to the children. Evidence: The LI (Licensing Inspector) observed laundry detergent and fabric softener in an unlocked wooden cabinet accessible to the children. Plan of Correction: The provider will remove the detergent until the cabinet can be locked.
