Discussed renewal of background checks, due May 2015 Discussed zoning, special use permit application has been submitted by the provider.
Provider is present with 3 children during this monitoring visit. A total of 5 points. The children are playing with blocks and puzzles and other toys. The provider is preparing lunch , the children will have ground turkey, mixed veggies, bread and applesauce today for lunch. Interaction between the provider and the children observed. The provider has lots of different interest areas set up. Please call me at 703-203-8659 if you have any questions regarding this visit. This visit was started at 11:45 and ended at1:00.
Standard #: 22VAC40-111-100-A Description: The provider failed to get physicals for the children in care. Evidence:Based on the review the children's records, two children do not have any physical forms in file. The third child has no record. Plan of Correction: I will ask the parents for the information.
Standard #: 22VAC40-111-60-A Description: The provider failed to get paperwork for one child in care. Evidence: Based on the review of the children's records, one child in care has no file. The child started with the provider Monday of this week. The provider does have the parents phone number. Plan of Correction: I gave the parent the paperwork on Monday and she came back today but did not have the paperwork because she did not have time to fill it out. I will make sure she gets the paperwork to me before the child returns for care.
