You and your assistant have until March 25, 2015 to complete your training hours for the year. Please send me documentation of your training hours when they are complete.
An unannounced renewal inspection was conducted today from 10:40 - 12:35. There were 6 children = 6 points with one caregiver. The licensee was not present during the inspection. The children played computer games, looked at books, played with blocks, colored, used math flash cards and ate lunch. The exit interview was conducted with the assistant caregiver. The violation notice will be mailed for completion within 10 days of receipt.
Standard #: 22VAC40-111-920 Description: Based on observation, the menu did not reflect the actual food served for lunch. Evidence - The menu stated that pizza, melon and milk would be served. The actual food served was pizza, applesauce and milk. Plan of Correction: The actual food served will be posted whenever there is a change in the menu.
Standard #: 22VAC40-111-60-A Description: Based on a review of children's records, the licensee did not update the record of one child. Evidence - The record for child #6 had not been updated by the parent since 4/2/2013. Plan of Correction: The parent will be notified and the record will be updated.
Standard #: 22VAC40-111-90-C Description: Based on a review of children's records, the licensee did not obtain an updated immunization record for one child. Evidence - The immunization record for child #6 has not been updated since 4/2/2013. Plan of Correction: Parent will be notified. The immunization record will be updated.
Standard #: 22VAC40-111-610-A Description: Based on observation, the use of television is not limited. Evidence - The television was on from the time the inspector arrived at 10:40 until 12:00 even though the children were not watching it. Plan of Correction: The television will be turned off.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diaper changing mat is no longer non absorbent. Evidence - The diaper changing mat has several small tears in the surface so that it is no longer non absorbent. Plan of Correction: The diaper changing mat will be replaced with a new one.
Standard #: 22VAC40-191-40-D-5-a Description: Based on a review of the records for family members, the licensee did not obtain all of the required documents for one family member. Evidence - The record for family member #4 did not contain a Child Protective Services Central Registry Check. Plan of Correction: The family member #4 has a Child Protective Services Central Registry Check.
