Comments:
An unannounced monitoring inspection was conducted today from approximately 11:00 to 1:30pm. Ten children, plus one resident child, were present with the provider, assistant and another adult who is helping out because the regular assistant is out today. Total 35 points with 3 caregivers. The children were observed eating lunch, and playing with toys. The infants were being fed, diapered, and sleeping. One assistant is new. Her record was reviewed today. Two children are new since the last inspection. Their records were reviewed today. The outside play area was not checked today because of snow. Violations:
Standard #: 22VAC40-111-770
Description: Based on observation, the provider failed to have a working battery-operated weather band radio and extra batteries available at all times. Findings: At the time of the inspection, the inspector found that the battery operated radio was not working, and there were no extra batteries available.

Plan of Correction: New batteries for the radio will be obtained, and extra batteries will be available.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to obtain all the information that is required to be in each child's record. Findings: At the time of the inspection, the inspector found that there was no written authorization for emergency medical care in the record of Child #2.

Plan of Correction: Will obtain the parent's written authorization for emergency medical care for this child.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider failed to obtain documentation of immunizations before a child attended the family day home. Findings: At the time of the inspection, the inspector found that there was no documentation of immunizations for Child #1, who started in care in October, 2014.

Plan of Correction: Documentation of immunizations for this child will be obtained.

Standard #: 22VAC40-111-170-A
Description: Based on record review, the provider failed to obtain a current Report of Tuberculosis Screening from each caregiver. At the time of the inspection, the inspector found that there was no documentation of a TB screening report for the new assistant who started over two months ago.

Plan of Correction: The assistant will obtain a current report of a TB screening.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface is nonabsorbent and washable. Findings: At the time of the inspection, the inspector observed that the diaper changing mat was torn in some places.

Plan of Correction: The diaper changing mat will be replaced.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review, the provider failed to obtain background checks for household member #1, who moved into the home over two months ago.

Plan of Correction: Background checks for this household member will be obtained as soon as possible.
