Comments:
An unannounced renewal inspection was conducted today, 3/19/15. The provider was present with four children ( 4, 6, 7 and 8 years of age). The children were engaged in free play ( legos, coloring, playing with toys) during the inspection. The provider does not administer medication. Three children' s files and the provider's files were reviewed. The provider's First Aid/CPR is through American Safety and Health Institute and expires on 2/8/17. Arrival time: 4:55 pm Departure time: 5: 45 pm

Standard #: 22VAC40-191-40-D-1-c
Description: Based on a review of record keeping, it was determined that one person has not obtained a repeat background check within 3 years as required. Evidence: Person A 's criminal record check is dated 10/13/2011.

Plan of Correction: I initially sent in the wrong form and I resubmitted the background check two weeks ago. It should be completed and returned to me in a month if not sooner.
