Comments:
An unannounced monitoring inspection was conducted today. Observed 8 children (16 points) directly supervised by the provider. Ratio and supervision were in compliance. A sample of 2 children's records, 1 caregiver record, and 1 household member record were reviewed. The physical plant and emergency procedures were also reviewed. No medications were reported to be on site. The children were engaged in various activities, to include: free play, singing, and workbooks. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Inspection conducted 10:00am-11:00am. Standard #: 22VAC40-111-800-B
Description: Based upon documentation reviewed, the emergency plan was not review annually and update as needed. Evidence: The family day home emergency preparedness and response plan was last reviewed on 09/01/2013.

Plan of Correction: I reviewed and updated.

Standard #: 22VAC40-111-60-B
Description: Based upon documentation reviewed, a child was incomplete. Evidence: One of the two children's records reviewed did not contain parent's signed acknowledgement of the receipt of information regarding the family day home liability insurance (Child A).

Plan of Correction: I corrected the missing the information. (The parent signed the paper) [sic]
