Comments:
An unannounced renewal inspection was conducted April 23, 2015 from approximately 10:00am to 12:30pm to determine the provider's compliance with licensing standards. In attendance were 10 non-resident children along with two resident children. The children were being supervised by two caregivers for a total of 28 points. When the licensing inspector arrived the children were playing outside with the caregivers. Children were observed playing with wagons, climbing on play equipment and running. Later children came inside and washed their hands and used the bathrooms. Diaper changes were also observed. Before eating lunch, children were observed coloring and watching Baby Einstein. Lunch consisted of fish filets, carrots, beets and milk. The provider?s home has three primary rooms for the children to use along with a bathroom. A complete walk through the home was conducted with the provider. The outside play area was also inspected. During the inspection the emergency supplies were inspected including the first aid kit, flashlight, and weather band radio. The emergency phone numbers along with a backup person?s name and phone number were posted. Last documented evacuation drill was documented as occurring on 04/10/15 and the last shelter-in place drill was documented as occurring on 02/17/15 The provider is MAT certified but the provider stated there are no children requiring medication at this time. Transportation is not provided at this time. Three children records, two caregiver?s records, and two household member's information were reviewed. Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation, the provider did not ensure all medications were kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection, the licensing inspector observed over-the-counter medication for the resident children in a cabinet over the kitchen sink. The cabinet was not locked.

Plan of Correction: Per provider - We will move the medication to the locked box located in the bathroom.

Standard #: 22VAC40-111-90-A
Description: Based on review of three children's records, one child's record did not contain documentation of immunizations. Evidence: During the inspection, the licensing inspector observed the record for Child #2 (enrollment date 04/13/15) did not contain documentation of immunizations. Before a child can attend a family day home, the provider must obtain documentation of a child's immunization.

Plan of Correction: Per provider - I believe the parent's have given me a copy of the immunizations but I have misplaced it. If I cannot find it, I will request another copy.
