An unannounced monitoring inspection was conducted on April 24, 2015. There were five children present at arrival totaling ten points on this date. The children were observed during storytime, playing freely, and watching children's programming. The menu indicated that today's lunch would consist of salisbury steak, rolls, pineapple, mashed potatoes and milk. No medications are being administered in the family day home. Transportation is not provided by the family day home. One child's record was reviewed during the inspection. The provider's record and one household member's record were reviewed.

Description: Based on record review, the provider did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on April 24, 2015, the record for Child #1 did not include documentation of the name, office address and telephone number of the child's physician; name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation; name and policy number of the child's medical insurance, if applicable; names of persons other than the custodial parents who are authorized to pick up the child; chronic physical problems, pertinent developmental information, and any special accommodations needed; and the names and addresses of previously attended child day care and schools attended by the child.

Plan of Correction: The form is in the process of being filled out by the parent.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, in two of two household records reviewed, the provider did not ensure that background checks were conducted before three years for every person living in the family day home who is required to have them. Evidence: 1. During the inspection conducted on April 24, 2015, the sworn statement (3/10/12), central registry finding (4/13/12) and the criminal history record check (4/20/12) for the provider had all expired. 2. The sworn statement (3/10/12), central registry finding (4/13/12) and the criminal history record check (4/10/12) for household member #2 had all expired.

Plan of Correction: Since I am renewing my license, I will get these filled out and mailed off immediately.
