Reviewed standard 22VAC40-111-430.

An unannounced monitoring inspection was conducted on April 28, 2015 from approximately 12:15 PM to 2:45 PM. Two assistants were present with nine children. Children were observed watching TV, freely playing, eating lunch and napping. Two children's records and two caregiver records were reviewed. Licensing Inspector reviewed supervision, activities, equipment, children/caregiver records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required positing.

Standard #: 22VAC40-111-120-B
Description: Based on 2 of 2 records reviewed, provider did not ensure caregiver records has documentation of required information. Evidence: On April 28, 2015, the record for caregiver #1 and caregiver #2 did not have documentation of completed references.

Plan of Correction: The provider will contact the references provide documentation of calls being placed and results from the reference.

Standard #: 22VAC40-111-320-E
Description: Based on record review, the provider did not ensure the fireplace inspection was done annually. Evidence: During the inspection conducted on April 28,2015, the last fireplace inspection documentation was conducted in 2010.

Plan of Correction: The provider will ensure inspections was completed and documentation will be available during the inspection.
