54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted with provider. There were six day care children and one resident child present totalling 14 points. The children were waking from rest, having afternoon snack, and playing with balls and bubbles in the back yard. Snack served today: oranges, goldfish, and water. Provider does not administer medication -Medication Administration Training is required to administer medication. Provider renewed first aid and CPR certification on 3/7/2015. Three children's files were reviewed - please see Supplemental Summary for missing items. Standard #: 22VAC40-111-60-B Description: Based on review of three children's files, the provider failed to have documentation of a second emergency contact address for each child. Evidence: Child B did not have an address for a second emergency contact. Plan of Correction: Provider will request this information from the parents.
Standard #: 22VAC40-111-70-A Description: Based on review of children's files, the provider failed to provide all of the required information in writing to the parents before the child's first day of attendance. Evidence; Child A did not have documentation of the VDSS model form titled "Provisions of the Emergency Preparedness Plan" or an equivalent. Plan of Correction: Provider will give the information to the parent to be signed.
Standard #: 22VAC40-111-280 Description: Based on observation, the provider failed to store potentially poisonous substances such as cleaning agents in an area inaccessible to children. Evidence: There was a container of Clorox Wipes (labeled "keep out of reach of children" and "Caution") on the half wall in the main day care area. Plan of Correction: Provider will store potentially poisonous substances out of reach of children.
