Today an unannounced monitoring visit was conducted with the Provider this visit took place in the morning between 9:14am-11:00 there were 3 children present for a total of 12 points within the supervision guidelines. The children were observed having morning free play, breakfast, blocks, balls, and in transition for lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance record and fire drill log were observed during this inspection. Standard #: 22VAC40-111-830-C
Description: Based on a review of the records, the provider did not have documentation of an monthly emergency evacuation drill for 2014. Evidence: The monthly fire drill posted did not include a fire drill for April 2015.

Plan of Correction: Provider stated that she forgot to document the drill and included the date.

Standard #: 22VAC40-111-960-E
Description: Based on observation of the bottles, 2 bottles were found without the child's name and date. Evidence: 2 infant bottles, did not include the child's name and todays date.

Plan of Correction: Provider labeled and dated the bottles during this inspection.

Standard #: 22VAC40-111-60-B
Description: Based on a review of the children records, one record did not include the parents work address. Evidence: Child #2's record did not include the father's work address and parents signature on the record.

Plan of Correction: Provider will ask the parent for the information.

Standard #: 22VAC40-111-80-A
Description: Based on a review of the children records, one record did not include documentation the the Provider verified the child's proof of identity. Evidence: Child #1's record did not include documentation of birth certificate was viewed and the number documented.

Plan of Correction: Provider stated that she will ask the parent to bring a copy to view today and will document it.
