54.1-3408 Provider must be MAT certified to administer prescription medication.

Today an unannounced monitoring visit was conducted beginning at 10:45am. There were 6 children (18 points) present with the Provider and two assistant caregivers. Children were observed playing with blocks and toys, getting diaper changes, and preparing for lunch. Children's and adult records were reviewed today. There are currently no children with medications enrolled at this time. This inspection ended at 12:20pm. Should you have any questions, please call.

Standard #: 22VAC40-111-120-B
Description: Based on record review, three caregiver records (#3,6 and 7) are missing completed required documentation for their record. Evidence: Caregiver #3 is missing emergency contact information. Caregiver #6 is missing emergency contact information, job title, and a filled out sworn disclosure statement within 30 days of their start date (9/25/14). Caregiver #7 is missing emergency contact information, job title, and orientation by the end of their first date of employment (4/30/15).

Plan of Correction: We will make sure they fill out these forms and place them in their records.

Standard #: 22VAC40-111-280
Description: Based on observation, hazardous supplies are on a table, accessible to children. Evidence: On the table upstairs in the infant area, and the area where older children come upstairs to eat lunch, is a table with the following supplies that are in reach of children: vitamins, spray adhesive, hand sanitizer, and spray paint.

Plan of Correction: Items were moved to an area inaccessible to children during the inspection. I will make sure these are locked away when they are not being used.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, three adult records have expired background checks that have not been updated within 3 years of the most recent results. Evidence: Adult records #1, 2 and 4 have search of the central registry and criminal history background checks last dated 5/7/12. No new background checks have been submitted.

Plan of Correction: We will submit both background checks for all adults immediately. We will renew them within 3 years of the most recent results.
