Comments:
An unannounced monitoring inspection was conducted beginning at 10:00 a.m and ending at 12:15 pm. with the provider. There were four children in care ranging in age from three months to four years old. Children were observed having free play during the inspection. An infant was observed being fed. After free play, children prepared for lunch. Lunch today is choice of chicken or hot dogs, baked beans, corn, apple sauce and milk. Two children's records, one provider record and one household member record was reviewed. The weekly menu was reviewed. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of household members records, the provider failed to obtain an updated Tuberculosis (TB) test or screening for all household members. Evidence: Household member B's last TB test was dated 4/1/2009. A new test or screening should have been conducted on or before 4/1/2011 and again on or before 4/1/2013.

Plan of Correction: Household member B did not reside here for a period of time, but has move back to the residence. An updated TB will be done by the end of the week.
