Comments:
Today an unannounced monitoring visit was conducted with the Provider-Mrs. Nguyen this visit took place in the morning between 8:00-10am there were 8 children with a total of 13 points within the supervision guidelines. The children were observed having morning snack/breakfast, milk free play playing with cars, arts and craft, soccer. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-130
Description: Based on observation of the caregivers and interactions with the children, one of the caregivers present was unable to speak, read, and write in English as necessary. Evidence: Caregiver #2, who was present with the children was unable to communicate with this inspector when asked a question. Very limited english provider was assisting.

Plan of Correction: Provider stated that she is just helping today because her assistant had a meeting this morning. Provider stated that she will send the assistant home since she only have 13 points.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the diapering surface, the pad was ripped/torn and no longer non absorbent. Evidence: The diaper changing pad was found ripped.

Plan of Correction: Provider was able to find another and will discard the other one.
