An unannounced monitoring visit was conducted on 6/12/2015 . There was one child in care at 1 point with the licensee. The inspector arrived at 1:45 and departed at 3:40. The inspector reviewed one child's records and the licensee's record. Observed the child involved with toys, work time, and rest time. Discussion with licensee about: night time care requires beds for children, storage of pesticides, obtain a crib that meets all CPS requirements for children under the age of one year. No medications are currently given.

Description: Based on interviews, the license failed to ensure that all children 12 months of age and under had a crib provided for them. Evidence: 1. The inspector interview the licensee and found that child# 2 started on 1/2015 and did not turn 1 until 4/15. The licensee stated she does not have a crib.

Plan of Correction: The licensee will not accept children under the age of 12 months unless a crib is purchased.
