An unannounced monitoring inspection was conducted from 9:24 a.m. - 11:25 a.m. There were eight children in care with the provider and then also an assistant. Two children's records were reviewed. The record for the assistant was reviewed. The children were observed singing songs from memory, working on the art project for Happy The Hippo, watching The Fox and Hound, talking with one another and their teachers, during diapering, hand washing and engaged in social skill activities. Standard #: 22VAC40-111-60-A
Description: Based on record review there was one child present who did not have a folder to review. Evidence: 1. On 07/14/2015 the provider advised she could not find the record for child #1.

Plan of Correction: This child has been in care for two years and there is a folder. I am confident that I will find it later this afternoon.

Standard #: 22VAC40-111-570-A
Description: Based on observation the provider failed to have another caregiver present when there were eight children in care equaling seventeen points. Evidence: 1. On 07/14/2015 the licensing inspector noted there were eight children in care totaling seventeen points with only the provider for approximately 5 to 10 minutes. 2. On 07/14/2015 the provider confirmed there were eight children present for a total of seventeen points for approximately 5 - 10 minutes in her care only until the assistant came into work.

Plan of Correction: Today is unusual as my regular assistant had to go to court so being without another caregiver should not be an issue in the future.
