Three children were in care during the initial inspection, total of six points. The age range of the children was three years to six years old. Children were playing in the day care area, ate lunch and read books. Provider is not current in MAT at this time. Inspection time 11:00am-1:15pm
Standard #: 22VAC40-111-770 Description: Based on observation not all emergency supples worked. Evidence: The weather band radio did not work during the inspection. Plan of Correction: I will buy a weather band radio.
Standard #: 22VAC40-111-60-B Description: Based on review of three children's records not all were complete. Evidence: Child A and B with a start date of 4/27/15 did not have documentation of mother and father's work address and phone number. Child C with a start date of 6/14/14 did not have documentation of one emergency contact's home address, The medical insurance number and mother and father's work address. Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-191-40-C-1-b Description: Based on review not all house hold members had background checks. Evidence: House hold member A and B Did not have a search of central registry check. Plan of Correction: I will send out the checks this week.
