Discussed with Provider: One crib (of 7 ) does not have documentation of an acceptable manufacture date and must be removed from the day care. It is not needed for use at this time. Gates for the upstairs area when in use.
An announced Initial study was conducted today. There are 5 children day care (12 points) in care today with one adult. 3 adult and 3 children's files were reviewed. See violation notice for missing information. Emergency supplies, Fridge temp, first aid supplies and diapering supplies are all in compliance. Standard #: 22VAC40-111-60-B Description: Based upon a review of files, one child's file is incomplete. Evidence: Child B's files does not contain documentation of a complete address for one of the emergency contacts. Plan of Correction: I will ask the parent to provide the address.
Standard #: 22VAC40-111-90-A Description: Based upon a review of files, one child's immunization record is not acceptable. Evidence: Child A's immunization record is not signed, stamped or electronically signed by the physician's office. Plan of Correction: I will ask the parent to provide to correct immunization record.
