An unannounced monitoring inspection was conducted today from 10:10 - 11:15. There were 6 children in care = 10 points with two caregivers. The children did math flash cards, colored, went outside for a walk and played with balls. The exit interview was conducted with the licensee. The violation notice will be mailed to the licensee's home for completion.

Standard #: 22VAC40-111-30-E
Description: Based on a review of the children's records, the licensee cannot ensure that the home's services are conducive to the welfare of one child. Evidence - The record for child #7 contains information that the child has a severe allergy and if an exposure occurs, needs an antihistamine which is not available.

Plan of Correction: I will notify the parent to bring the medication.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, the licensee did not obtain all of the required information for two children. Evidence - The record for children #7 and #8 do not include a second emergency contact.

Plan of Correction: I will notify the parent and obtain all of the required information.

Standard #: 22VAC40-111-80-A
Description: Based on a review of children's records, the licensee did not obtain the verification of identity for two children. Evidence - The records for children #5 and #6 do not include verification of the children's identity.

Plan of Correction: I will notify the parents to bring them.

Standard #: 22VAC40-111-220-A
Description: Based on information from the licensee, she does not have Medication Administration Training. Evidence - The licensee stated she does not have Medication Administration Training. She does have one child enrolled who has a severe allergy and needs an antihistamine if an exposure occurs.

Plan of Correction: I have registered for the training on August 8th.

Standard #: 22VAC40-111-580-B
Description: Based on observation, caregivers did not provide age - appropriate activities for children in care. Evidence - All of the six children present were observed seated at one table while a caregiver used math flash cards. The children ranged in age from 19 months to 11 years. The older children remained seated at the table for an hour until the licensing inspector suggested they go outside for a walk.

Plan of Correction: I will separate the old [sic] ages children from the young ones
