54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced focus inspection was conducted on Monday, August 3, 2015, from approximately 9:00am until 11:00am. Present today were seven children totaling twenty-three points with two caregivers. The children were playing when the inspector arrived. They were playing drums, rolling balls, having tummy time, playing musical instruments and dress up. The caregivers comforted and held the infant. Children were having conversations with each other and with the caregivers. Snack: Crackers and juice Lunch: sliced hotdogs, french fries, applesauce, whole wheat bread and milk A practice shelter-in-place drill was conducted 4/15/15. A practice evacuation drill was conducted 7/14/15. Eight children's records were inspected.
Standard #: 22VAC40-111-90-A Description: Based on inspection of children's records, the provider did not obtain documentation of immunizations before a child may attend the family day home. Evidence: Out of eight children's records inspected, Child #1, started 7/30/15, did not have documentation of immunizations. Plan of Correction: Per Provider: Child #1 temporarily attending but will obtain copy of immunizations.
