Comments:
Licensing Inspector arrived at 9:45 and departed at 11:00 AM. At the time of the inspection, there were eight children (ages infant through preschool) for a total of 27 points in care with two caregivers. Children were observed coming in from outdoor play, having story time and participating in free play. No medication is being administered or stored on site.

Standard #: 22VAC40-111-100-A
Description: Based on a review of four children's records, it was determined that the licensee did not ensure that a copy of documentation of a physical examination by or under the direction of a physician was obtained prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #1, date of enrollment 11/13/14, did not have documentation of a physical examination on file. 2. Child #3, date of enrollment 8/3/15, did not have documentation of a physical examination on file.

Plan of Correction: Provider contacted parents during inspection to obtain copies of physicals.
